HRSA Accreditation and Patient-Centered Medical Home Recognition Initiative
HRSA Accreditation and Patient-Centered Medical Home Recognition Initiative supports health centers in obtaining Ambulatory health care accreditation and/or Patient-Centered Medical Home (PCMH) recognition. PCMH recognition has become a standard of care for HRSA funded health centers. Ambulatory health care accreditation and PCMH recognition increase health outcomes, improve health equity, and lower costs for patients and health centers.

‍Ambulatory health care accreditation:
• Assesses health center quality and patient safety
• Evaluates health centers against standards of quality improvement, quality assurance, risk management, and performance improvement

‍PCMH recognition:
• Assesses a health center’s approach to patient-centered care
• Evaluates health centers against national standards for primary care that emphasize care coordination and on-going quality improvement.

Community Health Quality Recognition (CHQR) Overview

The Community Health Quality Recognition (CHQR) badges recognize Health Center Program awardees and look-alikes (LALs) that have made notable quality improvement achievements in the areas of access, quality, health equity, and health information technology for the most recent UDS reporting period. In addition, COVID-19 badges were introduced to recognize health centers’ contributions to the public health emergency response in the areas of data collection, testing, and vaccinations. To see if your health center received a CHQR badge:

• Go to the Health Center Program Uniform Data System (UDS) Data Overview page.

• Select “Health Center Program Type” (choose awardee or look-alike) and the state/territory using the filter tools.

• Using your browser’s scroll button, scroll down to view the list of health centers in the selected state/territory.

• Click “View Data” to see data for a specific health center. If a health center has earned CHQR badges, the badge images will be visible at the top of the health center’s data page.

Health centers that earned CHQR badges can request high-resolution badge images from Health Center Program Support .
